Rates and Insurance


Rates

My rates are reasonable so that I am able to accommodate all income levels.  These are general fees, but types of sessions and rates can vary:

  • $120 for a 50 minute individual therapy session.
  • $180 for an extended 80 minute individual session.

Fees or copayments are due at the time of each session. I accept cash, check or any of the major credit cards (AMEX, VISA, Discover, Master Card) and can provide you with an immediate receipt via email.

At times I am able to provide therapy on a sliding fee scale. Please inquire about this if you are not able to afford the full fee for service charge.

If you have an authorization for EAP sessions, there is no charge whatsoever to you or your dependents who receive EAP counseling with me.

Insurance Information

I am an in-network provider for many EAP companies and for MinnesotaCare.  Presently I am the in process of attempting contracting with two insurance companies. At this moment in time, I would be considered out of network (or a non par) provider.

We can discussion possible options: 1) if you have an HSA account, you could pay for sessions directly from your HSA; 2) you could pay me directly by check or credit card and submit a receipt to your insurance provider to be applied to your out of network insurance deductible.

It is important to contact your insurance company to ask the following questions:

  • What are my outpatient mental health benefits? Do I have coverage (for individual or couple) counseling?
  • What is the coverage amount per therapy session with an out of network LICSW?
  • How many therapy sessions does my plan cover each year?
  • How much does my insurance pay for an out of network provider?
  • Is approval or referral required from my primary care physician?
  • How do I submit a claim for reimbursement for an out of network provider?
  • Can I use my HRA or flexible spending account to pay for services?
  • Do I have a deductible? If so, is my deductible combined with medical and behavioral benefits or is it a separate deductible? Has my deductible been met?
  • Is there anything else I need to know?

If using insurance, I ask that you bring your insurance card and a photo ID at time of our first session.

I am happy to provide you with a receipt that you can submit for your flexible spending account if you have one. You may also qualify for reimbursement under another employee benefit plan, such as a HSA (Healthcare Savings Account), or a FSA (Flexible Spending Account). Reimbursement from a HSA often has fewer restrictions.  If you have a credit card connected to an HSA, I am able to accept payment through my Square iPad application.

Payment

Cancellation Policy

If you do not show up for your scheduled therapy appointment and have not provided me with at least 24 hours advance notice, you will be billed $75. 00 for your missed session.
